Title: DiaBetter Together: A Strengths-Based Peer Mentor Program to Support Young Adults with Type 1 Diabetes During the Transition from Pediatrics to Adult Care

Aim: Behavioral RCT testing the impact of a novel transition intervention that introduces peer mentors as a source of modeling, skills building, accountability, and social support for young adults with type 1 diabetes during the transfer between pediatric and adult care.
